A Personalized Care Plan Designed Just for Your Child & Family

At Giving Tree, we believe that no two children — and no two families — are the same. That’s why every child we serve receives a personalized care plan thoughtfully designed around their unique medical needs, daily routines, and family environment. Our team will submit all paperwork to GAPP.

Assessment & Approval

If your child qualifies, a clinical assessment will be completed to determine the level of care needed. This information is submitted for GAPP Medicaid approval. Once approved, we’ll coordinate services — so your child can begin receiving care at home. In special cases of denial, we are readily available to assist with the appeal process.
